As you know, we are going to do your initial consultation "over the phone".

However, to do so and make sure we don't miss something important about your situation, we need for you to fill out our intake form.

We understand that the form is long, but please rest assured that there is nothing asked that we don't need to know about. Our goal is to answer all your questions and to give you answers you can count on. To do so, we need for you to supply us with information that is both thorough and complete, to the best of your ability. The more thorough and complete your answers are, the more thorough and complete our answers and advice will be.

When you get to the end of the intake form, please make sure you click on the "SUBMIT" button. Doing so will send the form to our office so that we can refer to it when we talk to you over the phone.

Just so you know: If you don't understand a question, can't figure out what we are asking, or just don't know the answer, don't worry about it. Just do the best you can. We will take it from there. If need be, we can always get more information from you later on.
